We are pleased to inform you that UHealth is now offering the COVID-19 vaccine to eligible patients based on state and federal guidelines. Our records indicate that you are eligible to receive the vaccine and we would like to schedule your appointment.

The vaccine is being provided free of charge. We are offering the vaccine in phases based on risk criteria and availability of the vaccine. Please note, if you have family members who have not yet received an invitation, additional invites will be forthcoming as additional phases are implemented. Your invitation is non-transferable. We plan to eventually offer it to all of our patients.

As a result of new State of Florida requirements, only Florida residents can receive the COVID-19 vaccine. Prior to receiving the vaccine, you will need to provide the following:

  • A valid Florida driver’s license or a valid Florida identification card.
  • Seasonal residents who cannot provide a valid Florida ID must provide two documents that show proof of a Florida address, such as a statement from a financial institution, a rental agreement, or a utility bill received within the past two months, along with a photo ID.

Please do not schedule your vaccine appointment if you cannot show this proof of residency upon arrival for the appointment.

Vaccine supplies are limited and appointments slots fill up quickly. If appointment times are not available when you try to schedule, please continue to check back by calling 305-479-2VAX (2829).

Vaccines are currently being administered at 1951 NW 7th Avenue in Miami.

Depending on the vaccine available, you may need two doses of the vaccine. If a second dose is needed, you will be notified at your appointment and will schedule the appointment for your second dose before you leave your first appointment.
