The portal vein is the vein that transports blood from your stomach and intestines to the liver. It splits into the left and right portal veins, which then branch into a series of smaller blood vessels that deliver blood throughout your liver. A portal vein embolization blocks the flow of blood to an area. This procedure is done when a section of your liver needs to be removed due to liver cancer

The removal of a portion of the liver, known as a resection, is usually performed about three or four weeks after the portal vein embolization. The interventional radiology experts at the University of Miami Health System are experienced in performing these complex procedures.

Who is a Candidate?

If you have a tumor due to liver cancer that needs to be removed, your doctor may recommend a portal vein embolization to cut off blood flow to that area of your liver.

What to Expect

Prior to the procedure, your specialist may advise you to stop taking aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), or blood thinners for some period of time. The portal vein embolization is typically performed as an outpatient procedure (doesn’t require an overnight stay in the hospital). To start the procedure, you lie on your back on the surgical table. You receive medicine through a vein (intravenous) to help you relax, but you likely won’t be completely asleep for the procedure.

Your interventional radiologist injects a local anesthetic (numbing only the immediate area so you won’t feel pain) into the skin near your liver (upper right part of your abdomen). Then, they insert a needle into the skin using specialized X-ray imaging equipment and contrast dye to help guide the needle to the targeted area of the liver. The contrast dye helps to provide more detail in the X-ray image that is taken. The doctor inserts a catheter (long, flexible tube) through the incision and injects medicine that cuts off blood flow to this area.

Once complete, your interventional radiologist removes the catheter and needle and covers the incision with a bandage and sterile dressing. The portal vein embolization procedure usually takes a few hours. You can expect to remain in a recovery area for another few hours afterward. Usually, you can return home later that same day. In some cases, you may need to spend a night in the hospital for further observation.
