Y-90, also called radioembolization, is a procedure that combines embolization (deliberate blocking of blood flow) and radiation therapy to treat liver cancer. Y-90 involves injecting tiny beads filled with the radioactive isotope yttrium-90 (Y-90) into the blood vessels that feed the tumor, which blocks the blood supply to the cancerous cells.

Who is a Candidate?


If you have liver cancer and you’re not a candidate for other treatment options like surgery or liver transplant, your doctor may recommend Y-90 treatment.

What to Expect


Your doctor may ask you to stop taking aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), or other blood-thinning medicines before your procedure. You will also likely need to undergo a pre-Y90 angiogram and embolization before your treatment. Typically, Y-90 therapy is performed as an outpatient procedure (doesn’t require an overnight stay in the hospital) that uses local anesthesia (numbing only the affected area so you won’t feel pain).

Once the anesthetic has taken effect, your interventional radiologist makes an incision in the skin over your liver. Using specialized imaging equipment, they insert a catheter (thin, flexible tube) into your skin and guide it to the tumor. Once in position, your interventional radiologist uses the catheter to inject radiation-filled microspheres (tiny beads) into the blood vessels that feed the tumor. The microspheres deliver a concentrated, highly targeted dose of chemotherapy directly to the blood vessels, thus cutting off blood supply to the tumor.

Once the Y-90 beads are delivered, your interventional radiologist removes the catheter and places a bandage and sterile dressing over the incision. The procedure generally takes about an hour to complete.

You can expect to remain in observation for a few hours after the procedure for monitoring. Most people can return home that same day and resume normal activities within 24 to 48 hours. You may experience some mild side effects — such as nausea, vomiting, and fever — for three to five days after Y-90. Your specialist may prescribe medicine to help relieve symptoms.

Radiation from the Y-90 beads decreases over a two-week period and completely disappears after 30 days. The microspheres themselves remain in the liver, but they won’t cause any problems. You’ll receive detailed information about the treatment from your care team, and they can answer any questions.
