Frequently Asked Questions

How Does Billing Work? Page 1

Patient insurance information is required for third party billing. If self-pay, the case will be placed on hold until payment information is provided. Referring entity is responsible for obtaining pre-authorization from the payer if required. If the consultation request form is incomplete, the slides will not be reviewed. If payment is denied by the patient's insurance, the ordering institution/provider will be invoiced for the services and will be responsible for payment.

What Do I Ship? Page 1

Pathology report - Please include specimen source, collection date, and clinical history when available.

Specimen/Glass Slides - Please package slides and/or paraffin blocks to ensure the integrity of the specimen(s) during shipping, handling, and exposure to excessive temperatures.

Return of pathology materials will be by US Postal Service unless a FedEx or UPS account number is provided at the time of submission.

For questions regarding submitting samples, please call Pathology Customer Care at 305-243-7284 or email pathology@med.miami.edu.

How to Package and Ship Pathology Slides and Blocks Page 1

Ensure that your patient’s samples are received in the best condition by following these simple steps:

  • Place slides in a hard plastic box shaped slide container and tape the lid of the container. Do not use the flat cardboard style slide containers. Wrap the slide box in bubble wrap.
  • If sending a paraffin block, place the block inside a plastic specimen bag. To prevent melting during transit, a small refrigerant gel pack can be used. Use bubble wrap.
  • Include all materials and the completed Pathology Consultation Form in a padded envelope. Place the padded envelope into a padded express shipper (FedEx, UPS, DHL) envelope.
  • Recommended shipping services
    • FedEx Priority Overnight
    • UPS Ship Next Day Air
    • DHL Ship Express
    • Do not use U.S. Mail service
