For your safety and the safety of others, please do not come for your appointment if you have tested positive for COVID-19, been in close contact with someone who has tested positive, are showing symptoms or answer yes to any of the screenings questions below. If you meet any of these criteria, you may reschedule your appointment by calling 305-243-4000.
Please note that you will be asked these screenings questions again upon your arrival to the appointment.
