Your appointment to receive the COVID-19 vaccine has been confirmed. Please note that this appointment cannot be transferred or used for another person.
Per State of Florida requirements, only Florida residents can receive the COVID-19 vaccine. On the day of your appointment, please bring the following:
- A valid Florida driver’s license or a valid Florida identification card.
- Seasonal residents who cannot provide a valid Florida ID must provide two documents that show proof of a Florida address, such as a statement from a financial institution, a rental agreement, or a utility bill received within the past two months, along with a photo ID.
- Smartphone – to schedule your second dose appointment.
Please drive to Converge Miami, at 1951 NW 7th Avenue, Miami, 33136, for your COVID-19 vaccine appointment. Take a ticket at the entrance to the parking lot and someone will validate it for free parking.
