Leaving the hospital is a big step, but your healing is just beginning. The UHealth Total Care Clinic provides a follow-up visit 7–10 days after discharge to help you recover safely, catch problems early, and make sure you understand your care plan.

Condition Snapshot

A post-discharge follow-up visit helps keep you safe at home, supports a smoother recovery, and can reduce the chance you’ll need to return to the hospital.

Why You Should Come Back 7–10 Days After Leaving the Hospital

In the first days at home, your health can change quickly. A prompt follow-up helps your care team:

  • Catch small problems before they get bigger
  • Review your medications so you know what you’re taking and why
  • Answer your questions
  • Help arrange what you may need at home, like medical equipment, therapy, or community support

Many patients, about 1 in 7, need to come back after a recent discharge. Seeing a provider within 7–10 days can lower the chance of returning to the hospital by up to 40%.

Cost

There is no charge and no copay, because this visit is part of your hospital stay.

Telehealth or In-Person: You Can Choose

Yes, you can choose a telehealth visit or come in person.

An in-person visit may be better if you need:

  • Blood pressure checks, or
  • Blood sugar checks

Your comfort matters. Choose the option that works best for you.

“I Already Have My Own Doctor. Do I Still Need This?”

If you already have an appointment with your own doctor within 7–10 days, you may not need a Total Care Clinic visit. The goal is simply to make sure you’re seen quickly by someone on your care team.

What Happens During Your Total Care Clinic Visit?

You’ll meet with an experienced nurse practitioner, who will check how you’re healing and help ensure you get the care you need.

Your visit may include:

  • Checking your medicines (confirm you have them, and you know how to take them)
  • Checking how you feel (review symptoms and address what isn’t improving)
  • Ordering tests if needed (and helping you schedule them)
  • Answering your questions (bring any concerns, this visit is your time)
  • Reviewing home health services (confirm visits are set up and happening)
  • Helping with follow-up appointments (primary care and specialists)

Location

UHealth West Building
1321 NW 14th St, Suite 401A
Miami, FL 33136
