A Comprehensive Guide to Preserving Male Fertility

Are you considering fatherhood but want to wait? Or, do you have health concerns such as cancer that may affect your male fertility?

Sperm banking offers a fertility preservation option that can improve your chances of having a biological child. It allows you to freeze your sperm for future use, offering peace of mind and flexibility when you're ready to start a family.


What Is Sperm Banking?

Sperm banking involves collecting and cryopreserving (freezing) a semen sample, which can then be stored for years.

The purpose is to have quality sperm available in case your fertility changes in the future. Sperm cells are typically present in the semen that is produced during ejaculation. However, the number and quality of your sperm may change as you age. Also, some cancer treatments and other medical conditions may affect your ability to produce semen and sperm.

Sperm cryopreservation (freezing) and banking can:

  • Help preserve your chance of having a child if you have cancer or other medical or personal issues that can affect your future fertility.
  • Expand your fertility options, enabling you to use your sperm in procedures such as in vitro fertilization (IVF) and intrauterine insemination (IUI).

Who Should Consider Sperm Banking?

Sperm banking candidates may include men seeking male fertility preservation for different reasons. Taking proactive steps to preserve your sperm may help you start your family on your timeline.

Here are some examples of why you might choose sperm freezing:

  • Cancer and other health conditions. Certain conditions, such as testicular cancer, Hodgkin’s lymphoma, or leukemia, may reduce your fertility or possibly prevent you from having biological children. Also, treatments such as chemotherapy or gender affirmation surgery may decrease or possibly stop your sperm production. Male oncofertility services provide fertility care specifically for men with cancer.
  • Delaying fatherhood. Family planning or a focus on your career/education may cause you to delay starting a family.
  • Lifestyle choices. Certain professions or hobbies may carry risks to fertility.
  • Relationship status. Banking your sperm can give you options if you’re waiting to find a partner, or if your partner needs your sperm at a particular time for fertility treatments.
  • Age. The number of healthy sperm that you have naturally tends to go down with age.
  • Other reasons. Preserving your sperm can give you more reproductive options if you’re considering a vasectomy, or if you’re postponing having kids because you’re deploying to the military.

The Sperm Banking Process

The process of sperm freezing is straightforward and minimally invasive. A healthcare professional will guide you through the steps, which typically involve:

  1. Consultation. You’ll discuss your reasons for freezing your sperm with a health professional, who will ensure that you understand the process.
  2. Sperm collection. For this process, you’ll ejaculate in a private room and collect the semen in a provided container. Fertility specialists understand that this process may be difficult or against your personal beliefs. Talk with your provider if you have concerns, and they can help you find a solution that works for you.
  3. Sperm storage. After you’ve obtained your semen sample, lab technicians mix the semen (containing sperm) with protective liquids to prevent damage during the freezing and thawing process. Technicians then freeze your sample using a special technique and store it in a secure facility. They’ll also keep a small sample to test for information such as the total number and quality of sperm.

On the day that you arrive for sperm collection, you can expect the sperm banking process to take about three hours.


Benefits of Sperm Banking

Sperm banking can offer benefits such as:

  • Future fertility. Semen samples can be preserved for years when stored properly.
  • Increases reproductive options. You have more control over your family planning timeline.
  • Improves success rates in fertility treatments. Using frozen sperm can be highly successful in assisted reproductive technologies (ART) such as in vitro fertilization.
  • Preserves your genetic potential. Sperm banking provides a chance to have biological children after cancer treatment.
  • Offers flexibility. You can use the frozen sperm whenever you're ready to start a family. You don’t need to use it all at one time.

Risks and Side Effects of Sperm Banking

Sperm banking has been successfully used for decades. While the safety of sperm banking is well-established, it can sometimes raise concerns in certain cases. Possible sperm banking risks and complications may include the following:

  • Ejaculation issues. Some conditions or physical disabilities may cause erectile dysfunction or make it difficult to collect a semen sample. You may also have apprehension or discomfort about the semen collection process. Help is available. Talk with your provider and learn about fertility preservation options for males with disabilities.
  • Concerns about inherited risks. If you have cancer or other health conditions, you may worry about passing these on to your child. Some conditions run in families, and others don’t. Your doctor or a genetic counselor can answer your questions and address your concerns.

Success Rates of Sperm Banking

Sperm banking has been found to have successful fertility preservation outcomes for many people. But not all frozen sperm necessarily results in pregnancies. Sperm banking success rates can vary based on a number of factors, such as:

  • The treatment used (IVF or intrauterine insemination).
  • The age, health, and lifestyle of your partner or egg provider.
  • The quality of your banked sperm.

Another factor may be how your sperm survive the freeze-thaw process. Some sources say that one-half or more of sperm may die in this process.

Sperm Storage and Duration

When your semen sample is ready for storage, a technician will put it into small vials with a freezing solution. They then put the vials into special freezers that contain liquid nitrogen.

Sperm must stay frozen at a constant temperature to be properly preserved. Sperm storage facilities monitor the temperature and conditions to help ensure the safe long-term storage of sperm.

There is no set limit on the duration of sperm freezing. Not all sperm will survive freezing. However, cryopreserved (frozen) sperm have been found to be viable for decades.


Costs and Financial Considerations

Sperm banking costs can vary. In general, costs may range from about $500-$1,000. You can also expect to pay an annual storage fee to preserve and maintain your sperm until you’re ready to use them.

Insurance coverage typically does not cover sperm banking costs. Still, it’s important to check with your insurance to understand your benefits.

What Happens After the Sperm Is Frozen?

The next steps after sperm banking depend on you and your timeline. When you’re ready to use your sperm, a technician will remove your sperm from storage. They’ll then begin the process of sperm thawing until your sperm reaches room temperature. This typically involves placing the vials of sperm in a warm water bath as part of the thawing process.

The post-freezing process is typically done when you’re ready to use your sperm for a fertility procedure. As part of the thawing process, technicians can process the sperm for the type of procedure you have planned, such as IVF or intrauterine insemination.

Thawed sperm can usually only survive for up to 24 hours. Once thawed, they cannot be refrozen.
