Concierge medicine, also known as retainer-medicine, membership-medicine or subscription-medicine, is a relationship between a patient and a physician in which the patient pays an annual membership fee or retainer.
Yes. An annual membership fee will be required upon enrollment in the practice.

Yes. Specialist referrals will be made as necessary.
Most appointments can be accommodated within a timely manner of referral request. Based on the sub-specialty, some physicians have greater availability than others.
No. The concierge program is exclusive to the UHealth Premier physicians. The sub-specialists do not have concierge practices.
Yes. Physician visits, tests and studies will be billed through a patient’s insurance. Deductibles and co-payments are still the responsibility of the patient.
Yes. As the practice evolves, rates may be adjusted accordingly. However, the membership fee in place at the time a patient agreement is signed will be honored for an entire 12-month period.
Yes. MyChart or MyUHealthChart is an EPIC supported patient communication portal. All healthcare systems that use EPIC also use some version of MyChart for patients to access their electronic medical record and communicate with their healthcare team.
Usually yes. EPIC’s Care Everywhere feature allows healthcare systems to access patient health information (PHI) provided the patient signs a records release form with each institution. Information can be obtained electronically without the need for printing and mailing or faxing large volumes of documents between facilities.
For the most part yes. There are some parts of the medical record that are not yet releasable to patients via MyChart or MyUHealthChart. The documents scanned into your records from other providers will not be viewable on MyChart.
Yes. You may select the UHealth Premier physician. If the physician you chose has a full panel of patients, then you may select from the other UHealth Premier physicians who still have membership availability.
The UHealth Premier physicians will be available by phone seven days per week including weekends and evenings.
If your personal physician is away or unavailable, another UHealth Premier physician or nurse practitioner will be available to care for you until your personal physician returns.
