Patient referral
To refer your patient, please complete the following form. You may also call 305-243-9100 or email us.
To request an appointment, the following are needed:
- Patient’s name, address, telephone number, email address and date of birth
- If the international patient is an established UHealth patient, the Medical Registration Number (MRN)
- Type of insurance or financial arrangement: Self pay or insured (international insurance, travel insurance, and medical assistance companies)
- Diagnosis, health condition, symptoms, etc.
- Specialty and/or services being requested
- Name, care specialty, and contact information of referring physician
Please keep in mind that non-urgent appointments are scheduled days to weeks in advance. If your patient has an urgent health care or surgical need, please tell our UHealth International coordinators so that scheduling can be prioritized appropriately.
