Real-time survivorship care for cancer survivors
The My Wellness Check strives to understand the needs of cancer survivors and develop a comprehensive model of care that meets those needs, reduces cancer burden, and enhances quality of life.
My Wellness Check uses a Patient-Reported Outcomes (PROs) model to screen patients for physical, emotional, nutritional, and practical needs via computerized assessments linked in MyUHealthChart. By filling out a simple questionnaire, survivors can inform their care team about unique needs in real-time.
Once needs are identified, we develop a personalized, multidisciplinary care plan with the cancer disease group and supportive oncology services. My Wellness Check allows for continuous monitoring of patient assessment scores to improve patient and system-level outcomes.
Data generated will be used to assist care providers in delivering precision and patient-centered care. My Wellness Check also meets accreditation requirements set by the American College of Surgeons, Commission on Cancer (COC) 2020 Standards, Section 5.2.
