Planning for your future

You will leave with a personalized Cancer Survivorship Care Plan that has been created with input from your medical team. This care plan is be used as a resource to your other non-oncology providers so they are also aware of what has occurred in regards to your cancer diagnosis. You will also be provided with healthy living tips and a review of recommendations geared towards your general medical follow-up and care. Other important aspects in cancer survivorship relate to the importance of cancer screening to prevent future recurrence or new cancers, management of late effects related to treatment, and the aging process.

Frequently Asked Questions

When can I be seen in the Adult Survivorship program?
The program is open to all patients who have completed cancer treatment. We offer care for survivors of any cancer diagnosis, as well as disease-specific services.

When should I come to the Adult Survivorship program?
The UM Survivorship program is currently seeing patients within 6 months of completing active treatment. Patients may still be taking medications, such as hormonal therapies for breast or prostate cancer, and they are welcome in our program.

How can I be seen in the Adult Survivorship program?
To visit the Adult Survivorship Program, your Oncology healthcare provider should refer patients. Please discuss with your Oncology Healthcare provider if you have not been referred so that they can help facilitate the scheduling of your appointment.

Will my health insurance cover my visit?
In most cases, yes. University of Miami’s Survivorship program can help answer questions about payment for this visit. To learn more, contact SCCCSURVIVORSHIP@miami.edu

What is a Survivorship Care Plan?
This is a personalized care plan that gives you and all the doctors involved in your care a written document that details the treatments you received for your cancer. It also give a schedule of upcoming and recommended follow up visits going forward. This is a time when general healthy living tips and local and national resources and reviewed and information is given to you.
