What is My Wellness Check?
My Wellness Check is a program at Sylvester that provides cancer survivors with a personalized health care experience. By filling out a simple screener, you can inform your care team about all aspects of your wellbeing—from symptoms and financial concerns to nutrition and lifestyle. With your answers, your care team can address your needs.

How do I access the questionnaire?
Use the MyChart app on your smartphone or tablet, or visit MyUHealthChart.com. Sign in with the access code provided at your last visit or in your after-visit summary (AVS). If you cannot find your code, please contact 877-448-1773. Once logged in, go to your message folder to find the questionnaire.
What happens with my questionnaire results?
Your answers are sent to medical or supportive care teams. You may be contacted by phone to review your responses. If needed, your doctor will review your answers with you during your next scheduled appointment. This review process allows your care team to make adjustments to personalize and optimize your care at Sylvester.
How do I opt-out of My Wellness Check?
Please contact our My Wellness Check patient navigator at 305-243-8864 or MyWellnessCheck@med.miami.edu with your name, DOB, and MRN number, requesting to opt-out of the My Wellness Check screener.
