Physician and Provider Recruitment

We invite you to review the information below for an overview of actionable onboarding items that must be completed in a timely manner to ensure a successful start at the U. The Office of Physician & Provider Recruitment, along with your Department Chair and administrative staff, are here to support you throughout your journey.

Background Screening Page 1

You will receive an email from Sterling Infosystems, our background screening partner, shortly after accepting your offer. This email will request your authorization to complete a background check; please provide your contact information and any requested information on prior employment or education.

Credentialing & Privileging Page 1
You will receive a link via email from AppCentral to register and initiate the credentialing process & privileging processes. The application includes step-by-step instructions and a list of required supplemental documents to submit along with the application. Upon submission of the application and supplemental documents, a credentialing specialist is assigned and will provide you with personalized service to assist with completion of your application. Medicare/Florida Medicaid Provider Enrollment Page 1

Your Department will coordinate with you to complete the necessary Medicare and Florida Medicaid Provider Enrollment Applications. Please click here for more information.

Fingerprint Screening Page 1

You must complete a Level II background fingerprint screening, which is required for Medicaid provider enrollment. Fingerprint screening is offered on the Medical Campus or can be completed at an approved Florida location. Your Department will help schedule this appointment, but feel free to contact the Credentialing team at 305-243-5711 with questions.

Health Questionnaire Page 1

All faculty and staff involved in the care of patients through UMMG, UHealth, or affiliated hospitals and clinics must complete a health screening and provide verification of required immunizations to Employee Health or obtain these immunizations prior to initiating contact with patients.

  • Required immunizations include Hepatitis B, Influenza, MMR (Measles, Mumps, & Rubella), Varicella (Chickenpox), Tdap (Tetanus, Diphtheria, Pertussis), and TB Screening. In addition, annual influenza vaccination is a requirement of employment for all faculty and staff.

    At least two days prior to your health screening, please Download the Health History Questionnaire and email the completed form along with your immunization records to PhysicianClearance@miami.edu.

Prior to your start date, you will be required to attend a pre-employment visit on the Medical Campus. You will be contacted by a member of Physician & Provider Recruitment to schedule this half-day appointment.

  • Please bring with you, documents that establish both your identity and your I-9 employment authorization.
    Note: All documents must be unexpired and original.

Pre-Employment Visit Agenda

Human Resources Appointment

You will meet with a Human Resources Representative to Review your I-9 documentation.

Health Screening

You will complete a health screening at the Office of Employee Health Please ensure you have sent the required health questionnaire outlined above prior to your appointment.

*Follow Up Appointment: Please keep in mind that you may need to return 2-3 days after your initial appointment to have your TB Screening read, if applicable.

Badge Photo

You will meet with security to have your badge photo taken. Badges will be available for pick-up on your first day of employment.

Proof of Terminal Degree Page 1

Please request that your degree-awarding institution send an official, original grade transcript showing award of your terminal degree to the office of Physician Recruitment and Provider Recruitment at physicianrecruitment@med.miami.edu

CaneID & Email Page 1

About a month prior to your start date, you will also receive your CaneID username/password and have access to login to your University of Miami email at this time. You can access your UM email by navigating to email.miami.edu and logging in using your CaneID and password. Please note, you will be required to set up Duo, multi-factor authentication prior to logging in.

Workday Onboarding Tasks Page 1

About a month prior to your start date, once you have completed and cleared your pre-employment screening, credentialing, and background check, you will receive an email from our payroll system, Workday, to complete some additional onboarding requirements. These will include:

  • Policy Acknowledgments
  • W4
  • Bank Information-Direct Deposit
  • I9
  • Benefit Elections

Faculty Initial Appointment Page 1

If applicable, The Office of Faculty Affairs and Professional Development will partner with you and your Department to initiate the appointment process. You will receive additional information regarding the process via an email sent from the Faculty Affairs APT Inbox, facaffairsapt@miami.edu.

Department-specific Onboarding Tasks Page 1

Your Department Chair and administrative team will assist with department related onboarding tasks such as ordering lab coats and business cards, arraigning for a professional headshot, EMR access/training, and completing your kyruus physician profile. Please do not hesitate to contact them with questions.
