Cervical Cancer Questionnaire

Complete our questionnaire to see if you meet the criteria for a cervical cancer screening.

1. Have you had a total hysterectomy (removal of the uterus and cervix)?

2. Do you have one or more of these symptoms?

  • vaginal bleeding after sex
  • vaginal bleeding after menopause
  • vaginal bleeding between periods or periods that are heavier or longer than normal.
  • vaginal discharge that is watery and has a strong odor or that contains blood.
  • pelvic pain or pain during sex
  • difficult or painful bowel movements or bleeding from when having a bowel movement.
  • difficult or painful urination or blood in the urine
  • unusual pain in the abdomen

Generally women who have had a total hysterectomy (removal of both the uterus and cervix) do not need to have a Pap test or HPV test unless the hysterectomy was completed to treat cervical cancer or a precancerous growth.

Generally, a cervical cancer screening is not recommended if you received adequate prior screenings that were consistently negative according to guidelines. Nearly all cervical cancers are caused by HPV. If you are under the age of 21, talk to your doctor about getting an HPV vaccine. Learn more about HPV.
