We use input from your medical team to develop your survivorship care plan. It offers comprehensive guidelines to support your health and well-being – from tips for healthy living to recommendations for general medical care and preventive screenings. It includes information about managing treatment side effects, healthy aging, and more. Your plan also informs providers outside your cancer care team about your cancer diagnosis and treatment.
