Survivorship Care for Your Body, Mind, and Spirit

We endorse the National Coalition of Cancer Survivorship’s definition of a survivor: You’re considered a cancer survivor from the time of diagnosis through the rest of your life. Family, friends, and caregivers are also part of the survivorship experience.

Cancer and its treatment-related side effects can affect your quality of life. At Sylvester, we understand cancer survivors’ physical and emotional needs and how expert guidance can make a positive impact. That’s why we provide comprehensive survivorship services that address the whole person – body, mind, and spirit.

Understand Long-Term Side Effects of Your Cancer Treatment

After cancer treatment, you probably have questions and concerns – particularly about long-term and recurrent side effects. When you come for your first cancer survivorship visit, you’ll meet with your survivorship care team. It includes nurse practitioners, clinical nurse specialists, and physician assistants who work closely with your care team and develop a cancer survivorship care plan.

Your cancer survivorship care team will answer all your questions and talk with you about the physical, emotional, and practical issues cancer survivors often face. We provide resources for exercise and nutrition and access to support groups, seminars, workshops, as well as various therapies, such as art, music, and massage, and more.

A Survivorship Care Plan for Your Needs

Your cancer is unique, so is your road to recovery.

Your personalized cancer survivorship care plan is created just for you, with input from your medical team. Your plan includes healthy living tips and recommendations for your general medical care and follow-up guidelines. It also provides guidance on cancer screening to prevent recurrence or new cancers.

Share this plan with providers outside your cancer care team to make sure they know about your cancer diagnosis and treatment.

Your Health and Wellbeing is Our Priority

As an NCI-designated cancer center, we’re continually seeking better ways to enhance cancer treatments and the quality of life of cancer survivors.

One of the ways we enhance your survivorship care experience is through My Wellness Check. Before your clinic visit, use MyUHealthChart, to complete a brief questionnaire to inform your care team about all areas of your well-being – including side effects, symptoms, or concerns like transportation or childcare.

Your questionnaire responses help everyone. With your participation, we can study the social, economic, and physical effects of cancer so that we can improve care for cancer survivors of all ages.

Depending on your needs, we also provide specialized survivorship care through our wellness clinics.
